Forms

Online forms for churches: visitors, registrations and requests

You build the form from fields and steps, share the link or the QR code, and see the responses on the dashboard. Each response can create the person’s record and open a task.

How it’s built

The form grows as you design it

People filling it in see a short form, with only the questions that apply to them.

  1. 1

    Fields and steps

You choose the type of each field and whether it’s required. Long forms are split into steps.

  2. 2

    Conditional questions

A field appears only when the answer to an earlier question is the one you specified.

  3. 3

    Consent and sending

The form asks for consent to process the data and links to the privacy policy.

Files and signature

Permission slips and documents without going through email

A file field accepts PDF, JPG, PNG or WEBP up to 5 MB. The signature field asks for the full name, which counts as the signature and is stamped with the date of submission.

  • The file opens in the response, on the dashboard, for people with permission.
  • The signature is the full name typed out, not a hand-drawn signature.
  • Up to 50 fields per form, with or without steps.

What happens when someone responds

The team gets a notification for every response. You add the actions that make sense.

Create or link the person

With “Create visitor”, the response creates a record, or links to the existing one with the same email or phone number.

Open a task

A task for the team to follow up, with the title you set.

Add a label

Tags the person with a label, such as “Visitor”, so you can find them later.

Alert the team

An in-app alert, even if the usual notifications are switched off.

More control

When, who and how many times

“I’m new here” template

A ready-made visitor form. You review the fields and share the QR code.

Opens and closes

You set when it opens and when it closes, and the maximum number of responses.

Members only

Only people who are logged in and have a record in the church can respond.

Anonymous responses

The response isn’t linked to a person, even if they’re logged in.

Responses and statistics

You see each response and the daily totals, and you can export to CSV.

Questions

Frequently asked questions

Is the signature drawn by hand?

No. The signature field asks for the full name, which counts as the signature and is stamped with the date of submission.

Which files can I receive?

PDF, JPG, PNG or WEBP, up to 5 MB per file. The limit is shown on the field itself.

Can I show a question only in certain cases?

Yes. Each field can appear only when the answer to an earlier field is the one you chose, such as Yes on a checkbox or an option from a selection.

How is data consent handled?

The public form shows a privacy notice with the option to accept the processing of data and a link to the policy. Without acceptance, the response isn’t sent.

Does every response create a person record?

Only if you turn on “Create visitor”. In that case the response creates a person or links to the existing one with the same email or phone number. On an anonymous form this doesn’t apply.

Which plan includes forms?

Forms are available on every plan.
